For people that need a transition between hospital care and taking care of themselves at home, Wellington Place rehab-to-home short term rehabilitation program is the solution. Generally, people who have a short-term rehab stay at our center are recovering from a surgery, accident or illness, and benefit from physical, occupational or speech therapy services.
For example, a person fell in her home getting into the shower. After a hospital stay and short trial at home, they came to Wellington for Skilled Nursing and Rehabilitation because the stitches in the area of the surgery had become infected and she wasn’t able to care for herself independently.
She is not alone. According to the CDC about one third of the elder population over the age of 65 falls each year, and the risk of falls increases proportionately with age. At 80 years, over half of seniors fall annually. Falls account for 25% of all hospital admissions and 40% of all nursing home admissions.
