We want to be your 5 star! Yes, to be sure we are providing Kind, Compassionate, Care and that we are the 5-star facility you deserve, we need your help. We want to be your 5!

In our continuing efforts to identify those areas in which we may better serve our residents we are conducting periodic surveys. An independent firm will manage this exercise on our behalf and they have been asked to collect and evaluate those surveys in a confidential manner. It is critical that everyone receiving a survey responds in a timely manner.

The purpose is to gather date that will enable us to identify areas we may need to improve on so you will always be proud to say that your experience with us was 5 stars! I would appreciate it very much if you would take a few minutes needed to complete these surveys when you receive them and return them in the self -addressed, stamped envelope.
